I will be in California during the course of treatment I am LEGALLY able to consent for therapy for myself or my child I am able and interested in meeting WEEKLY via zoom or in person in Sunnyvale, CA I understand collaboration and transparency is part of successful treatment and I agree to informing Justine of other providers on the team and big life changes that could impact work. I read all the boxes and checked the ones that apply I just like clicking boxes and I wanted to move on so I clicked them all ;)
